All documents and forms must be submitted to the Honors College
The Honors College will primarily be accepting forms via email and Blackboard. Please review the submission instructions for each form to determine where to submit. Students can find these electronic forms by scrolling down to the forms below or by visiting the Honors BlackBoard and locating the Documents & Forms link within the Honors Information module on the organization's homepage. Honors documents submitted virtually must be filled out, scanned, and submitted via Blackboard or email at HONORS@SHSU.EDU. Please note: WE DO NOT ACCEPT images. Only scanned documents or PDF forms. Our forms are fillable PDFs, so students also have the option to submit digitally via email.

Honors Online Documents and Forms
Course Contract Proposal - It is VERY IMPORTANT that you double-check all the information being filled out, most specifically your email address and your professor's email address. If either of these are input incorrectly, you will not receive updates regarding your contract status, nor will your professor be able to continue with it. Contact the Honors College (email@example.com) immediately if you have provided incorrect information.
Community Service - Download and save the PDF fillable form and wait to submit it until complete. We do not accept incomplete Community Service forms.
Graduation Declaration - The Honors College Graduation Declaration form is ONLY for students graduating in the fall 2023 semester. Remember, you must also apply to graduate with the university itself.
|New Student||Participation Agreement||
The Participation Agreement is required from all accepted students. This form is the official acceptance of Honors College membership and must be submitted in order to begin receiving the benefits of being in Honors. Please email signed PAs to firstname.lastname@example.org.
A Course Contract is to be completed by students enrolled in non-Honors courses seeking Honors credit. For more information, read Contract FAQs or Contract FAQs for Faculty.
The Contract Proposal Form must be submitted via Blackboard prior to beginning a course contract. Proposal forms are due by the fifth class day.
The Contract Certification Form must be submitted via Blackboard after completing a course contract.
The Contract Cancellation form must be submitted to the Honors College if you decide to cancel your contract before the Contract Certification Forms become due. Please note that this may result in being placed on Status two (probation) with Honors.
Note: If neither of the Proposal or Certification forms is submitted, the student will not receive Honors credit for a course contract.
Community Service is to be completed by students seeking Honors Advance Registration for the following semester. For more information, read Community Service Facts.
The Community Service Form must be submitted via Blackboard upon completion of at least 10 hours of service per semester.
Note: If the form is not submitted by the deadline listed, the student will not be considered for advance registration.
The Honors Thesis is to be completed by students wishing to graduate with Highest Honors. For more information, read Thesis FAQs.
The Thesis Proposal Form must be submitted prior to beginning an Honors Thesis.
The Thesis Template is designed to help guide students in the layout and design of their Honors Thesis.
The Thesis Final Approval Form and Thesis Binding Form must be submitted after the completion of an Honors Thesis.
Note: If neither form is submitted by the deadline listed, the student need to fill out the Thesis Withdrawal Form and will not be considered for graduation with Highest Honors.
The Graduation Declaration is to be completed by students wishing to graduate with Honors.
The Graduation Declaration must be submitted during the semester in which a student hopes to graduate.
Note: If the form is not submitted by the deadline listed, the student will not be considered for graduation with Honors.
|Seminars||Seminar Wait List Request Form||
An Honors Seminar is to be completed by students wishing to stay on track to graduate with Honors. All Honors students are required to enroll in and successfully complete two Honors Seminar courses.
The Seminar Wait List Request Form is for students seeking enrollment in an Honors Seminar that were not able to register for one regularly via the Registration Portal.
The Withdrawal Form is for students wishing to end their membership with the Honors College.
The Request for Letter of Recommendation Form is for students seeking a letter from an Honors College Dean. The form must be submitted at least two weeks prior to the requested completion date.
