Microsoft Forms - Creating a Form

You can access Forms by logging into office.com with your SHSU credentials and selecting the Forms icon from the menu on the left or you can log into the website directly at forms.office.com using your SHSU credentials.

Once logged into Forms, you can create a new form or quiz by selecting an option from the drop-down menu on the Forms home page. When you create your form or quiz, you can also create branching options which allows you to customize the flow of the form or quiz.

Create Form or Quiz

  • New Form

    If New Form is selected, a new form will display. Select the area that says Untitled form to start building the form.

    Name Form

    Once you select the area for the Form name,

    1. Enter the form name.
    2. Enter a description of the form.
    3. Select + Add new to add the first question.

    When you select + Add new, you can select what type of question you want to ask. Options include:

    • Choice: multiple choice question
    • Text: response will be a text answer
    • Rating: rate answer by using stars, numbers or smiley symbols
    • Date: response to question is a date
    • Ranking: rank items by arranging items in order of preference
    • Likert: uses a rating scale for various questions
    • File upload: allows a file to uploaded to a question (not available if form is shared external to SHSU)
    • Net Promotor Score: rate the likelihood of recommending a product or service

    Question Type Options

    Once you select the question type, you can create your question and review the options for each question (based on the question type). Below is an example of choice question options.

    Once the question is added, the completed question will display.

  • New Quiz

    If New Quiz is selected, a new quiz will display. Select the area that says Untitled quiz to start building the quiz.

    Once you select the area for the Form name,

    1. Enter the form name.

    3. Select the question type to add the first question.

    3. Select which answer is the correct choice (for choice questions. Will vary depending on question type).
    4. Assign the number of points to the question.
    5. Select for question options.

  • Branching

    Microsoft Forms offers branching for forms and quizzes. Branching allows the flow of the form or quiz to be customized based on responses to questions. To create a branching rule, go to the question that as the criteria in it for branching.

    1. Select the ellipsis () to open the question options.
    2. Select Add branching.

    In the Branching options, select how you want the form to branch based on the responses of the question. Below is an example of choice question options.
