Personal Information Updates

Both current and former students can use this form to change their name or social security number with SHSU. If you are a former student, you can use it to change your address too. We will also need additional documents from you: Instructions for those are below!

How to Change Your Information

To change your social security number, you will need to print and sign the form linked above.

You will also need to provide a copy of your social security card upon submission. You can send us your form and social security card copy in-person or through mail. If you wish to submit your form over email, you can send it to regforms@shsu.edu, and we'll reach out to you to set up a Zoom meeting so we can see a copy of your social security card: We do not encourage sending your social security card over email or fax as the security of your information can not be guaranteed. 

Social security cards are required for social security number changes to verify and remain in compliance with IRS records.

If you are applying for graduation this semester, do not delay submitting your name change. Deadlines do apply for your name to appear in the commencement program. However, if you miss the program deadline, you can still submit your name change and have it applied to your official graduation record and diploma. 

To change your name, you will need to print and sign the form linked above.

You can request to change either your full legal name (last, first, and middle) or your preferred unofficial first name. Preferred name changes only apply to your first name, are not an official name change, and will not display on official documents.

If you are requesting a legal name change to fix an issue with spacing, capitalization, or punctuation, you will need to provide a copy of your state-issued ID alongside your form. This can be delivered in-person, by mail, or through emailing regforms@shsu.edu with your documents. 

For all other legal name changes, you will need to provide a copy of your social security card upon form submission. You can provide us with your form and social security card in-person or through mail. If you wish to submit your form over email, you can send it to regforms@shsu.edu, and we'll reach out to you to set up a Zoom meeting so we can see a copy of your social security card: We do not encourage sending your social security card over email or fax as the security of your information can not be guaranteed. 

If you do not have a social security card, you must provide a copy of your passport; birth certificate; or another signed, legal court document instead. We cannot accept marriage licenses for name changes.

You will need to provide both your old address and new address on the form. Additionally, you will need to send us a copy of your state-issued ID alongside your form. This can be delivered in-person, by mail, or through emailing regforms@shsu.edu with your documents. 

For current students, permanent, mailing, emergency, and W2 addresses can all be changed online through MySam. We do have a guide on how to update your address available
