If you are a current student, please see our guide to change an address or update emergency contacts.
If you experience trouble with a W2 address change, please contact the Payroll Office at 936.294.1909.

To apply to graduate, access your Student Profile on MySam and select "Apply to Graduate" from the options on the left.

If you qualify for a disabled Texas firefighter or peace officer exemption, please fill out this form and email it to regforms@shsu.edu. More information on disabled Texas firefighter and peace officer exemptions can be found here.
To order a duplicate diploma, please fill out this form.
If you would like to withhold your directory information, please fill out our FERPA Opt-Out form. If you have previously submitted a FERPA Opt-Out and would like to have your directory information released again, submit another copy of the FERPA Opt-Out.
If you need to cancel your graduation application, please fill out this form.
Current students may access their IRS Form 1098-T electronically through MySam.
If you need to change your name (legal or preferred) or social security number, fill out this form and submit it to regforms@shsu.edu.
Please see our personal information change webpage for details on additional documents you may need to submit. Social security number changes and some legal name changes will require a copy of your social security card before processing.
To request a letter of non-attendance from SHSU, fill out this form. Communication regarding your request will go to the email you provide in your submission.
If you qualify for a peace officer and firefighter exemption, please fill out this form and email it to regforms@shsu.edu. More information on peace officer and firefighter exemptions can be found here.
To request to resign, please view our resignation information and resignation decision tool. Under the "Submit Resignation" header, selecting the "Resign" option will present you with the resignation decision tool, which will take you to the resignation form upon completion.
If you experience difficulty with submitting the online form, please send an email to the Registrar's Office at regforms@shsu.edu to request to resign. Please email from your SHSU email account and include your SAM ID, the term you wish to resign from, and an acknowledgement of the resignation policies.
If you qualify for a senior citizen waiver, please fill out this form and email it to regforms@shsu.edu. More information on the senior citizen waiver can be found here.
To submit documentation of any TSI exemption you may qualify for, fill out a TSI exemption form.
To allow us to access your TSI scores from tests taken somewhere other than SHSU, fill out a TSI score request form. If you tested through the SHSU Testing Center, a score request form is not necessary.
In your graduating semester, you can submit a tuition rebate form. More information on the tuition rebate can be found here.
Current students: Please utilize National Student Clearinghouse or submit a verification of enrollment request. Make sure to review the options listed on our enrollment verification information webpage before requesting a verification of enrollment.
Former students: Please utilize National Student Clearinghouse or request an official transcript.
