Curriculum Forms

The following forms should be used for curriculum requests.  As these forms may be updated periodically, please, access the forms through this web site each time a request is made, rather than saving the forms permanently to your computer or T-drive. Note: For analytic support for new program development (minors, certificates, degree programs), please, contact the Program Analytics Team by email or by phone at (936) 294-2291.


New Course Addition Request

A New Course Addition Request form is to be used to support each request for a new course.  The form must be completed in its entirety.  An incomplete form will not be eligible for review and will be returned to the department/college for resubmission.

New Core Course Addition Request

The Core Curriculum Course Proposal form is to be used to request changes in the baccalaureate core.

New Minor Request

The New Minor Request form is to be used to propose an undergraduate minor (Note:  All course curriculum must be part of the THECB approved SHSU course inventory prior to an approved minor being added to the catalog or Degree Works).
Note: Before starting this form, please submit an Program Analytics Request Form to begin the New Minor Development process.

New Certificate Request

The New Certificate Program form is to be used to propose an undergraduate or graduate certificate.  (All courses associated with the certificate must either already be in SHSU's course inventory or have been submitted to the Office of Academic Planning and Analysis and are going through the curriculum approval process simultaneously with the proposed certificate).

Before starting this form, please submit an Program Analytics Request Form to begin the New Certificate Development process.

New Degree Program Request (Estimated cost less than $2 million)

The New Degree Program Proposal: Stage I Needs Analysis form is to be used to propose a new bachelor or master's degree program that is not 1) in Engineering and/or 2) does not have an estimated cost of more than $2 million in the first five years of operation. The New Degree Program: Stage I Needs Analysis proposal form is the first stage in a three-stage---I Needs Analysis; II Financial Analysis; and III Content and Quality---review/approval process with the appropriate workflows designated between each stage.
Note: Before starting this form: Please, submit the Program Analytics Request Form to begin the New Program Development process.

The New Degree Program Proposal: Stage II Financial Analysis form is to be used to propose a new bachelor or master's degree program that is not 1) in Engineering and/or 2) does not have an estimated cost of more than $2 million in the first five years of operation. The New Degree Program: Stage II Financial Analysis proposal form is the second stage in a three-stage---I Needs Analysis; II Financial Analysis; and III Content and Quality---review/approval process with the appropriate workflows designated between each stage.
Note: Before submitting this form to Program Development: A New Degree Program Proposal: Stage I Need Analysis form must have completed the review/approval process with all required signatures

The New Degree Program Proposal: Stage III Content and Quality form is to be used to propose a new bachelor or master's degree program that is not 1) in Engineering and/or 2) does not have an estimated cost of more than $2 million in the first five years of operation. The New Degree Program: Stage III Content and Quality proposal form is the third  stage in a three-stage---I Needs Analysis; II Financial Analysis; and III Content and Quality---review/approval process with the appropriate workflows designated between each stage.
Note: Before submitting this form for University Curriculum Committee review, both a New Degree Program Proposal: Stage I Need Analysis and New Degree Program Proposal: Stage II Finance Analysis form must have completed the review process with all required approval signatures.

New Degree Program (Expanded) Request (Estimated cost more than $2 million)

The New Degree Program Proposal: Stage I Needs Analysis is to be used to propose a new Bachelor’s or Master's degree program (1) that is in engineering; (2) that has an estimated cost of more than $2 million in the first five years of operation; and/or (3) that does not meet the certification requirements set forth in Texas Administrative Code (TAC), Title 19, Chapter 5, Subchapter C, Section 5.44 (a) (3). The New Degree Program: Stage I Needs Analysis proposal form is the first stage in a three-stage---I Needs Analysis; II Financial Analysis; and III Content and Quality---review/approval process with the appropriate workflows designated between each stage.

Course Deletion/Change Request

A Course Deletion/Change Request form is to be used to request the deletion of or changes(s) to an existing course. A single form should be prepared at the department level and should encompass a summary of all course changes requested. The form contains a brief justification for course deletions and changes in course prefix, title, number, and/or combination of

Minor Deletion/Change Request

A Minor Change Request form is to be used for existing programs intending to a) request a name change; b) request an increase or decrease in total semester credit hours, and c) request a minor deletion. Please, note that a justification is required for all minor change requests.

New 4+1/3+2 (Existing Programs) Change Request

A New 4+1/3+2 (Existing Programs) Change Request form is to be used to request the combining of two existing programs—one baccalaureate plus one master’s—with a minimum of 150 (xx Baccalaureate + xx Master’s) semester credit hours.

Concentration (Undergraduate) or Track (Graduate) Change Request

A Concentration or Track Change Request form is to be used for existing programs intending to a) add a new concentration/track, or b) delete a concentration/track from an existing degree program.

Degree Program CIP Code Change Request

A CIP Code Change Request is to be used to request a change to the Classification of Instructional Programs (CIP) code of an existing degree program, meaning the degree program must already be on SHSU's program inventory.
Note: This request also requires the submission of a Board Motion, upon request.

Degree Program Title Change Request

A Title Change Request form is to be used to request a change to the Classification of Instructional Programs (CIP) code of an existing degree program, meaning the degree program must already be on SHSU's program inventory. Note: This request also requires the submission of a Board Motion, upon request.

Degree Program Semester Credit Hour (SCH)Change Request

A Semester Credit Hour (SCH) Change Request form is to be used to request a change in the number of semester credit hours (SCH) required for a degree program already on SHSU's program inventory. Note: This request also requires the submission of a Board Motion, upon request.

Administrative Unit Change Request

A Administrative Unit Change Request form is to be used to request a change to an academic administrative unit. Administrative changes include the creation of new administrative units--colleges, schools, divisions, departments--as well as changes to existing administrative units, such as a name change, consolidation of existing units, or movement of a program into another unit. Note: This request also requires the submission of a Board Motion, upon request.
