Curriculum Forms

The following forms should be used for curriculum requests.  As these forms may be updated periodically, please, access the forms through this web site each time a request is made, rather than saving the forms permanently to your computer or T-drive. Curriculum Forms

The Course Deletion/Change Request form is to be used to request course deletions and changes in course prefix, title, number, or a combination of.  

The New Course Addition Request form is to be used to request a new course.

The Course Description Update Request form is to be used to request a course description change to an existing course in the SHSU course inventory.

The Writing Enhancement Supplement form is to be used to request the designation of Writing Enhanced to a proposed new course addition.

The Core Curriculum Course Proposal form is to be used to request changes in the baccalaureate core.

The New Minor Request form is to be used to propose a minor. (Note: All courses associated with the minor must already be in SHSU's course inventory or going through the approval process simultaneously with the proposed minor).

The New Certificate Program form is to be used to propose an undergraduate or graduate certificate.  In addition to this form, the University Curriculum Committee requires the submission of a Certificate Narrative, which contains the following information: description/overview, justification of need, target demographics, admission requirements, and curriculum.  (Note: All courses associated with the certificate must already be in SHSU's course inventory or going through the approval process simultaneously with the proposed certificate).

The New Degree Program Proposal form is to be used to propose a new bachelor or master's degree program that is not 1) in Engineering and/or 2) does not have an estimated cost of more than $2 million in the first five years of operation. The New Degree Program Proposal form is a three stage form---I Needs Analysis; II Financial Analysis; and III Content and Quality---with the appropriate workflows designated between each stage. Note: As the new degree program proposal transitions into a 3-phased approach, Paige Mertz (936-294-4013), the Educational Program Analysis, is the contact for related questions and meeting scheduling as well as the resource for data gathering, analysis, and research.

The THECB New Degree Program Proposal form is to be used to propose a new bachelor or master's degree program only if the program is 1) in Engineering and/or 2) has an estimated cost of more than $2 million in the first five years of operation.

The Online Program Request form is to be used to request that a new program or an existing program be delivered either electronic-to-individuals (i.e., online), electronic-to-groups, or off-campus face-to-face. (Note: This request also requires the submission of a Board Motion.)

The New Doctoral Program Proposal form is to be used to propose a new doctoral degree program.  Please, be aware that the Texas Higher Education Coordinating Board requires notification of intent to offer a new doctoral degree a minimum of one year prior to the submission of a proposal. 

The New Online Doctoral Program Proposal form is to be used to propose a new online doctoral degree program.

The Title Change Request form is to be used to request a change to the title of an existing degree program.  The degree program must already be in SHSU's program inventory.  A degree program title consists of the following two parts: 1) degree designation, such as Bachelor of Science (BS), Master of Arts (MA), or Doctor of Philosophy (PhD) and 2) name of the discipline, such as History, Mechanical Engineering, or Zoology.  (Note: This request also requires the submission of a Board Motion.)

The CIP Code Change Request form is to be used to request a change to the Classification of Instructional Programs (CIP) code of an existing degree program, meaning the degree program must already be on SHSU's program inventory.  (Note: This request also requires the submission of a Board Motion.)

The Semester Credit Hour (SCH) Change Request form is to be used to request a change in the number of semester credit hours (SCH) required for a degree program already on SHSU's program inventory.  (Note: This request also requires the submission of a Board Motion.)

The Administrative Unit Change Request form is to be used to request a change to an academic administrative unit.  Administrative changes include the creation of new administrative units--colleges, schools, divisions, departments--as well as changes to existing administrative units, such as a name change, consolidation of existing units, or movement of a program into another unit.  (Note: This request also requires the submission of a Board Motion.)
