Concerns and complaints

In accordance with Texas Health and Safety Code § 181.005, SHSU's Counseling Center provides you direct access to the following resources:

• Report an ethical issue or legal concern with our licensing board.
• If you believe that you have a consumer complaint regarding the privacy and security of your health information, you may file a consumer complaint with the Texas Attorney General's Office.

Record request

Counseling records request must be done in writing. Include your full name, date of birth, Sam ID number, date(s) of service and the type of record you're requesting. Submit your request by emailing counseling@shsu.edu or in person at the William Powell Student Health & Counseling Center, 1608 Avenue J, Huntsville, Tx 77340.

We will contact you to confirm your request. Requests are completed within 10 business days of confirmation.
