How to request your health records
Existing and past clients may request health care records by contacting the office via email at counseling@susanehenderson.com or through the Simple Practice portal, if it is still active. Requests must be made in writing.

To file a complaint or ask question of unethical behavior, you may contact the Texas Behavioral health Executive Council, the disciplinary and licensing authority. All inquiries can be started by following the link https://bhec.Texas.gov/contact-us/ To file a complaint, after following the link, select Professional Counseling and/or Marriage and Family Therapy as the license, then complaints and enforcements.
