The following forms should be used for curriculum requests. As these forms may be updated periodically, please access the forms through this web site each time a request is made, rather than saving the forms permanently to your computer or T: drive.
Course Changes (Additions/Changes/Deletions)
- Curriculum Report Summary (Form A)*
- Request for Addition of a New Course (Form B) Revised February 2011
*Course changes (prefix, title, or number) and course deletions do not have a separate request form. These requests should be included on the "Curriculum Report Summary - Form A."
Modification to the Courses Listed in the Core Curriculum (Form C)
Degree Program Changes (Additions/Modifications)
