As these forms may be updated periodically, please access
the forms through this web site each time a request is made,
rather than saving the forms permanently to your computer
or T: drive.
Course Changes (Additions/Changes/Deletions)
*Course changes (prefix, title, or number) and course deletions
do not have a separate request form. These requests should
be included on the "Curriculum Report Summary - Form
A."
