Information Sheet to be provided to a Physician or Hospital for the Purpose of Verifying a Student’s Resignation Request

Students who have been incapacitated by an injury or illness requiring extensive recuperation may file a resignation request with the University Registrar and provide whatever written verification is required describing the student’s condition. The verification must be provided by a physician or hospital on an appropriate letterhead and should include the following information:

Brief description of the patient’s condition
Dates of the injury or illness
Brief description of the length and nature of recuperation The written verification may be submitted by the student or it can be mailed or faxed to:

Student: Please indicate the dean of your academic major. Correspondence for students with an undeclared major, general studies, should be directed to Dean, College of Arts and Sciences.
