Steps for Payment for Teacher Monitors

 

Step 1:

Complete and sign W-9 form.  This will be mailed or given to you by the Coordinator.  You only fill this out once.  The original must be in the TxCAE office so you have to mail or give to the coordinator.

Step 2:

Sign the contract for Teacher Monitor. [Online] This can be faxed to the center at 936-294-4874 Attn: Linda Creighton

 

Step 3:

Fill out and sign the “Request for Payment” form. [Online] This can be faxed to the center at 936-294-4874 Attn: Linda Creighton

 

Step 4:

Please complete Teacher Monitoring Record with appropriate signatures by May 31st of the current academic year for payment.
