Agreement for Contractual Services
This agreement, between Sam Houston State University, hereinafter
referred to as the University, and __________________, hereinafter referred to
as the Contractor, is entered into on the date signed below.
Now therefore, in
consideration of the mutual undertaking herein contained, the parties agree as
follows:
- The services to be performed by the Contractor
for the University shall consist of
- Minimal skill with PowerPoint program is
essential. No more than 40 slides per tutorial lesson.
- All lessons must be approved before being paid.
- Each lesson should include but is not limited
to, at least one objective and TEK. The subjects areas covered are Math,
Reading and Writing.
- Lessons can be geared for competency in grades 1
– 6 to allow for different difficulty levels.
- Only those lessons completed and used on the web
site will be paid for.
You must fill out appropriate forms and go
through the Sam Houston State payroll process for payment of services
- After the total and satisfactory completion of
services outlined in Paragraph 1 above, the University agrees to make
payment to the Contractor at a rate not to exceed $250.00. The Contractor will be required to
complete and sign the University’s proper payment forms before payment
will be made. After satisfactory
completion of the work covered under this agreement and receipt of
properly completed forms, The University will tender payment within
approximately forty-five (45) days. The University and the Contractor retain the unilateral right
to terminate with advance notice the services of the Contractor at any
time during the agreement period.
- In the event that this work is being done on a
grant project, it is mutually understood that this agreement will be in
force and have effect only upon the conditions that the grant or contract
is awarded to the University, and that both the University and Contractor
shall abide by the terms and conditions set forth in the grant or contract
documents, and that this agreement may be declared null and void by the
University if the granting agency, for any reason, withdraws the financial
support from the University.
- By accepting this agreement, the Contractor
certifies that dual compensation is not involved (i.e., the individual may
not receive compensation from his/her regular employer and the retaining
grantee or subgrantee for work performed during a single period of time
even though the services performed benefit both).
- All right, duties, and obligation under this
contract are subject to the laws of the State of Texas. In case of
conflict, dispute, disagreement, or interpretation of this contract, the
laws of the State of Texas shall control.
