This Program enables faculty to refer students who are doing poorly in class or who have frequent or unexcused absences. To refer for other reasons, please explain in the box on the form.
The process is simple; just fill out the form below and submit it. We will acknowledge receiving it and begin the process of contacting the student to offer assistance. You will be notified as to the outcome of our efforts.
It is most helpful if you make referrals early in the semester, especially before the last date to drop classes. That way, our intervention can be more effective.
If you have a long list of referrals, you may choose to photocopy your class list, highlight the names of those being referred, and let us know if these are for absences or low grades. This can be sent to us by campus mail. You may also refer students to us by phone or by using a paper First Alert form which can be sent through campus mail or dropped off. Please let us know if you wish to have pads of these forms.
