User Documentation - the IRB Form System

Click one of the following links for instructions on using the SHSU IRB form system for completing your IRB application: 

IRB Application (Classroom Activity) - Non-Exempt Human Subjects Research

Who can complete this application? (1) Any student conducting non-exempt Human Subjects research, but the goal is not completing the research for a Thesis, Dissertation, or other publication/presentation; (2) Any faculty member sponsoring a research project assignment in which the entire class is conducting non-exempt Human Subjects research that are considered less than minimal risk studies, and you are not interested in using the data/results of your study for publication/presentation. 

IRB Application (Thesis/Dissertation) - Non-Exempt Human Subjects Research

Who can complete this application? Any student conducting non-exempt Human Subjects research and the goal is completing the research for a Thesis or Dissertation. 

IRB Application (Regular Initial) - Non-Exempt Human Subjects Research

Who can complete this application? (1) all faculty and staff conducting non-exempt Human Subjects research; (2)Any student conducting non-exempt Human Subjects research for publication/conference presentation, but the goal is not completing the research for a Thesis or Dissertation. 

IRB Exempt Application (Classroom Activity)

Who can complete this application? (1) Any student conducting research involving Human Subjects that is exempt from IRB review*, but the goal is not completing the research for a Thesis, Dissertation, or other publication/presentation; (2) Any faculty member sponsoring a research project assignment in which the entire class is conducting exempt Human Subjects research that are considered less than minimal risk studies, and you are not interested in using the data/results of your study for publication/presentation. 

IRB Exempt Application (Thesis/Dissertation)

Who can complete this application? Any student conducting research involving Human Subjects that is exempt from IRB review*, and the goal is completing the research for a Thesis or Dissertation. 

IRB Exempt Application (Regular Initial)

Who can complete this application? (1) all faculty and staff conducting research involving Human Subjects that is exempt from IRB review*; (2)Any student conducting research involving Human Subjects that is exempt from IRB review* for publication/conference presentation, but the goal is not completing the research for a Thesis or Dissertation.

*Institutional policyrequires that the IRB must determine whether or not a study qualifies for an exemption based on federal regulations (45 CFR 46.101(2)(b)).

New CITI Instructions for Faculty and Students:

Academic Policy Statement on Ethical Conduct in Research

A general course in the Responsible Conduct in Research (RCR) training is available on the CITI website. PIs conducting research that DO NOT involve the use of Human Subjects or Animal Subjects, and they are submitting a grant to a federal granting agency (i.e., NSF, NIH, etc.) are required to take this course. Please follow the instructions to register for and complete the training: CITI Training - RCR.

The IRB Guidebook was last updated in 1993. Developments over the intervening years have made portions of the Guidebook information obsolete, while portions of the information remain valid. There is no errata document to indicate which information has been superseded. OHRP cautions users to verify the current validity of any Guidebook information before relying on the information in a program of human subjects protection.
