Returning Hazlewood students should complete the Hazlewood Application and the "Request for Information" form.

New or transferring Hazlewood students must complete the Hazlewood Application and "Request for Information" form and turn in a copy of DO-214 with a letter from VA stating you have exhausted or are not eligible for GI Bill.
