Please attach the following to your application before submitting it to our office:

To print your SHSU Unofficial Transcript, click here (you'll need to log into Sam Web)
To print your Lone Star College Unofficial Transcript, click here (you'll need to log into My Records then click on "My Advising Profile")

Proof of Income: You may submit a copy of your Student Aid Report if you completed the FAFSA or most recent income tax statement to verify taxable income.

Please complete FAFSA's Dependency Status Worksheet (quick online questionnaire)

If the results of the Dependency Status Worksheet indicate Independent, you are not required to provide a copy of your parents' most recent income tax return. (You may submit your income tax return along with your application)
If the results of the Dependency Status Worksheet indicate Dependent, you are required to provide a copy of your parents' most recent income tax return.

Your Student Aid Report (SAR) is basically a copy of your submitted FAFSA. It takes a few days to process your FAFSA after you have submitted it. After your FAFSA has been processed, you can view and print a copy of your SAR.
How to Print a Student Aid Report

Go to the FAFSA website: www.fafsa.ed.gov
On the website, on the right, you will see "FAFSA Follow-up". The last choice on this list is "View and Print your Student Aid Report" - click on this link.
Choose the appropriate year and click "next".
Enter your personal information and PIN and click "Submit".
On this page you will see one or more transactions. Choose the most recent transaction number by clicking on it.
You will see a menu on the left-hand side of the page. Click "Print Summary" at the bottom of this list.
You will be given two options. Click on the second option "SAR (PDF Version)".
Print the report and attach it to your application.

You can return the application to us in one of several ways,

Bring it by our office in the Teacher Education Center, room 339 (SHSU campus)
