Checklist for Applicants:
1. Apply to D&D Digital Systems (dnd@netins.net) for computer matching by February 15.
2. Complete the Graduate School application process. The application form and other information are available on the website at http://www.shsu.edu/~grs_www/.
3. Provide proof of scores from Graduate Record Examination (GRE).
4. Provide official transcripts from each undergraduate/graduate school attended.
Follow the Graduate School application (http://www.shsu.edu/~grs_www/forms/gradadm.html) instructions and deadlines and carefully mail the application and non-refundable fee of $20.00 before February 15 to:

5. Apply to the Dietetic Internship Program at Sam Houston State University on the American Dietetic Association’s Application Form. Applications MUST BE postmarked by February 28th. This application form and other pertinent information and materials are available on the following website: www.shsu.edu/~hec_www/
6. Evidence of entry into ADA’s match at D&D Digital Systems.
7. Two letters of recommendations.
8. Personal letter of application with statement of goals and professional interests.
9. Completed verification of Declaration of Intent to complete or completion of DPD academic requirements to be SIGNED BY THE COLLEGE DPD DIRECTOR.
10. A non-refundable internship application fee of $30.00 must be included with this application. Checks should be made payable to Sam Houston Sate University.
