Correspondence Forms
If you are a non-SHSU student you may submit your application through ApplyTexas.org and there will be an undergraduate admission fee of $45.00. You can pay the application fee as you complete your application through ApplyTexas.org. If you need assistance with this process you may contact Undergraduate Admissions at 936.294.1882 or 936.294.1889.
Once you have completed the application you will receive notification of your admission and at that point you may register to enroll in a correspondence course by using the registration form in the Forms section of this page. Please mail your registration form along with a copy of your letter of acceptance to the address listed on the upper left-hand corner.
If you need additional assitance enrolling in a correspondence course please contact Elizabeth Monteagudo at 936.294.3824 or by email at cor_emm@shsu.edu. You may fax your registration form to 936.294.3703 or scan it to the aformentioned e-mail address.
How to Make Payment
-Pay Online -
• Students pay through My Sam. Link to the "My Account" tab and access your balance by term
• Parents pay through the new Authorized User
access in the Student Account Center (If you previously had access to the
Parent BearKat Account, new access will have to be
granted by your student to the Student Account Center.)

Above are all the forms you will need from enrollment to extension requests. They can be opened in either Adobe Acrobat Reader, MS Word 97 and above, or in your browser. In either case, the form should be printed and submitted as directed.
