PROCEDURES TO RECEIVE SERVICES
Steps for receiving accommodations.
Request for accommodations must be initiated by the student. A student seeking accommodations should come to the Counseling Center and Services for Students with Disabilities in a timely manner. Please allow up to thirty (30) days for documentation to be reviewed.
- The student makes an appointment at the SSD office with the Coordinator.
- The student brings the appropriate documentation to the appointment.
- The Coordinator reviews the documentation.
- The student and the Coordinator will discuss the accommodation requests.
- The student and the Coordinator complete a notification form to be taken to each instructor by the student.
- The student delivers the forms and discusses accommodations with their instructors. After the forms are signed by the instructor, the original and Dean's office copies are returned to the SSD office by the student. At that time, accommodations will go into effect.
- Every semester that the student desires accommodations, it is the student's responsibility to complete a Classroom Accommodation Request Form at the SSD office and follow the above procedure in notifying faculty.
Untimely requests may result in delay, substitution, or denial of an accommodation. However, if a request is submitted late, SSD will make every reasonable effort to accommodate the request. |
