NEW BSN STUDENT CHECKLIST – SPRING 2018 COHORT 14

Congratulations, future Bearkat nurse! New BSN students will be required to complete and submit the following documents by specified due dates. Read each item on checklist carefully.  ALL email correspondence with the SHSU nursing program will be utilized through the student’s SHSU email account only. 

FAILURE TO COMPLETE STEPS BY STATED DUE DATE WILL CAUSE APPLICANT’S SEAT TO BE RE-ASSIGNED.

Email Acceptance of Invitation and Change of Major  forms to nursing@shsu.edu. Forms can be hand delivered to Nursing Advisor in Huntsville (CHSS 170) or The Woodlands Center (TWC 140). Due Date: Tuesday, September 19, 2017 at 5:00 PM. 

After the School of Nursing has received all Acceptance of Invitation and Change of Major forms, new students will be given access to Project Concert. Read introduction and instruction documents before accessing. 

Make an appointment with a healthcare professional to complete a physical and obtain a signed Health Certificate Form. Upload form to Project Concert by Due Date: Wednesday, November 1, 2017 at 11:59 PM.

  • Latex Allergy - If a student has a latex allergy, prior to the first week of classes, the student must alert their faculty of record for each clinical course and the School of Nursing Clinical Coordinator, Christine Teel (cteel@shsu.edu).
  • SHSU Student Health Center in Huntsville can see current SHSU by appointments or walk-ins

Complete Remaining Immunizations (if applicable): Hep B series and/or positive surface antibody titer. See Apply Now page for immunization PDF link. Upload records to Project Concert by Due Date: Wednesday, November 1, 2017 at 11:59 PM.

  • Note: If Hep B series or titer will not be able to be completed due to compliance with minimum intervals, the student must notify nursing@shsu.edu by due date.

 Obtain a current “Basic Life Support (BLS) for providers" card, certified by the American Heart Association*. A course can be scheduled here and this is an example of what the card should look like. Upload a copy of the AHA BLS card (front and back) to Project Concert by Due Date: Wednesday, November 1, 2017 at 11:59 PM.

 Student must have Health Insurance that is effective the first day of class. Upload a copy (front and back) of health insurance card to Project Concert by Due Date: Wednesday, November 1, 2017 at 11:59 PM.

  • If student's name is not on the insurance card, a verification of coverage must be obtained from the insurance company. 
  • Health insurance is available through the Student Health Center. If using this plan, the student must notify nursing@shsu.edu by Due Date: Wednesday, November 1, 2017 at 11:59 PM and proof of student health services insurance is extended to Friday, January 12, 2018 at 11:59 PM.

 Complete New Student Information Form (use Google Chrome web browser when opening form) and upload to Project Concert by Due Date: Wednesday, November 1, 2017 at 11:59 PM.

  • SAVE THE DATE: White Coat Ceremony, Friday, February 2nd, 2018 at 10:00 AM in Huntsville. 

 Upload a copy of Driver’s License or State ID (front and back) to Project Concert by Due Date: Wednesday, November 1, 2017 at 11:59 PM.

  The student will be notified via email when to register for electronic Fingerprinting* and Background Check. After fingerprinting is complete, upload a copy of the receipt to Project Concert. Due Date: Wednesday, November 1, 2017 at 11:59 PM.

  Once the criminal background check is complete, they will receive a Blue Post Card from the Texas Board of Nursing (TBON). Upload this post card (front & back) to Project Concert immediately upon receipt. Due Date: ASAP and/or prior to first day of class.

  • Note: Receiving a blue post card can take up to 90 days to complete, even with no issues. Do not delay in submitting the blue card. Without this card a student cannot attend clinical rotation.

□ Attend mandatory Uniform Fitting & Immunizations Clinic at The Woodlands Center on Friday, November 10, 2017 in room 439. Students will be receiving TB Skin Test* #1 and Flu Shot (if applicable). Huntsville students are assigned to be there 11:00 AM – 1:00 PM and The Woodlands Center students are assigned to be there 1:30 PM - 3:30 PM. If a student has class during their assigned time, they will need to notify nursing@shsu.edu to attend the other allotted time slot. Failure to attend will cause applicant’s seat to be re-assigned. BRING SHSU STUDENT ID CARD. If you do not have an SHSU student ID card, you can get one at SHSU TWC One Stop Shop.

□ Go to The Woodlands Center (details TBA) OR go to the SHSU Student Health Center (SHC) to receive TB Skin Test #1 Reading on Monday, November 13, 2017. The SHC prefers appointments but welcomes walk-ins. The student must go on this date for an accurate reading - See Apply Now page for immunization PDF link for details. BRING SHSU STUDENT ID CARD.

 Go to The Woodlands Center (details TBA) OR go to the SHSU SHC to receive TB Skin Test #2 on Friday, December 1, 2017. The SHC prefers appointments but welcomes walk-ins. BRING SHSU STUDENT ID CARD.

  • Students who cannot receive their TB Skin Test #2 on this date, they will be responsible for getting their second test completed and read with the 3-week guideline. See Apply Now page for immunization PDF link with details.

 Go to The Woodlands Center (details TBA) OR go to the SHSU SHC to receive TB Skin Test #2 Reading on Monday, December 4, 2017. The SHC prefers appointments but welcomes walk-ins. See Apply Now page for immunization PDF link for details. BRING SHSU STUDENT ID CARD. Upload records of both TB screenings to Project Concert by Due Date: Friday, December 8, 2017 at 11:59 PM.

□ The student will be subject to a randomized Drug Screening* before the first day of classes. After drug screen is complete, that student will have 48 hours to pay for the drug screening and upload a copy of the receipt to Project Concert. Due Date: As soon as instructed.

□ Register for J1 semester classes AFTER receiving an email from the School of Nursing with specific Course Registration Numbers (CRN). Due Date: As soon as instructed.

  • Note: Do NOT register for any nursing courses until instructed to do so. The nursing advisor will not be sending the CRN’s until after the November 1, 2017 due date. You may enroll or stay enrolled in non-nursing courses (up to 15 hours) as a holding place for financial aid purposes.

 It is recommended to purchase & receive J1 Textbooks (list to be uploaded) before January 2, 2017. Students will also need a functional laptop through out the entire nursing program. Students will not need to purchase the following items as they are provided for you: uniform/scrubs, shoes, watch, stethoscope, and all lab equipment.

 Complete School of Nursing Orientation Blackboard Modules. An email will be sent with timeline of modules at a later date. Recommended Timeline:  January 2 - January 15, 2017.

 (If applicable) Final Transcripts for all transfer college work (not previously received by SHSU) will need to be submitted to SHSU Undergraduate Admissions and emailed to prenursing@shsu.edu. Transcripts sent to Undergraduate Admissions will need to be official (electronic or sealed envelope) and transcripts sent to School of Nursing (SON) can be unofficial (PDF or screenshot attachment). These transcripts should include recently completed prerequisite grades for courses that were in-progress (Summer 2017) at the time of the application deadline. Due Date: Friday, January 12, 2017 at 11:59 PM.

  • Note: SHSU School of Nursing will need to be made aware of any course(s) where grades will not be posted by the due date by emailing prenursing@shsu.edu with a screenshot of enrollment dates.  Please note that a student's conditional acceptance is predicated upon satisfactory completion of all nursing program prerequisites.

Attend mandatory School of Nursing Orientation at The Woodlands Center Tuesday, January 16, 2018 and Friday, January 19, 2018. Huntsville students are expected to be at the joint orientation with their The Woodlands Center classmates. An email will be sent with specific times and an agenda at a later date. While we may not keep students the entire time, plan to be available for the orientation days from 8:00 AM - 5:00 PM. Failure to attend will cause applicant’s seat to be re-assigned.

*Finger-Printing/Background Check, Drug Screening, & Current AHA Basic Life Support (BLS) certification expenses are student’s responsibility.

Proof of Meningitis Vaccination

SHSU students will be required to submit proof of having received the Meningitis A vaccination before being able to register for the upcoming semester. Please note, if you have already submitted your Meningitis A form to the Student Health Center and have not been notified that your form was not accepted for incomplete data, you should be able to register for classes as soon as registration opens.
